Abstract

Congestive Heart Failure (CHF) is a condition where the heart fails to pump blood to meet the needs of body cells for adequate nutrients and oxygen. Pharmacological therapy performed on therapeutic treatment of patients with CHF can allow the occurrence of Drug Related Problems (DRPs). The identification of DRPs using the Strand Classification includes several criteria, namely indications without drugs, drugs without indications, wrong drugs, insufficient drug doses, excessive drug doses, adverse drug reactions, and drug interactions. This study aims to determine the incidence of DRPs in the therapy of hospitalized patients with congestive heart failure at H. Adam Malik Medan Hospital. This study uses the Retrospective method by collecting data on inpatients with CHD at H. Adam Malik Medan Hospital starting from July-December 2020 in patient medical record data. The data was analyzed using IBM SPSS Statistics. The results of the identification of DRPs in the therapeutic management of patients with the Main Diagnosis of CHD without complications and comorbidities in the hospitalization installation of H. Adam Malik Medan Hospital showed that out of 10 patients, there were 9 patients (90%) who experienced DRPs and 1 patient (10%) who did not experience DRPs. Consisting of indications without drugs 2 cases (11.8%), drug interactions 15 cases (88.2%), while drugs without indications, inappropriate drugs, less drug dose, more drug dose, and adverse drug reactions were not found (0%). It can be concluded that there is an incidence of DRPs in the therapy of Congestive Heart Failure patients at the H. Adam Malik Medan Central General Hospital for the period July - December 2020.
